Integrating primary care
Systems thinking in primary care supports an integrated model of care. That is, a model of primary care that is collaborative, and which builds capacity through partnership within a complex adaptive system – rather than one that is siloed. It has been recognised, at a policy level, that ‘[P]artnerships between professionals, across sectors and including consumers, strengthen the capacity of organisations to improve both individual and population health and reduce health risks.’
